Applications will be accepted through PhORCAS, the web-based pharmacy residency application system, starting in November. The deadline for application is January 8th, 2021.
Interested applicants are encouraged to contact the Residency Program Director, Paige.Porter@lpnt.net or Residency Program Coordinator, Carly.Feldott@lpnt.net prior to the ASHP Midyear Clinical meeting to express interest in the program.
Pre-requisites for PGY1/PGY2 two year combined programs:
Doctor of Pharmacy degree from an ACPE accredited school or college
Minimum GPA of 3.0
Eligible for Tennessee licensure
Tennessee licensure must occur by September 1 of the PGY1 residency year
All residency programs will be utilizing ASHP's Pharmacy Online Residency Centralized Application Service.
Application Process: 1. Submit Online: Applications must be submitted electronically via the ASHP PhORCAS portal.

Three letters of recommendation should be completed in the reference section within PhORCAS. Traditional letters of reference are not required.
3. Interview: Applicants will be considered for an interview only after all required application materials have been submitted. A 20-30 minute presentation will be required during the on-site interview.
